Frequently Asked New Patient Questions
Our goal at Optimal Health Spectrums is to provide you with comprehensive, personalized medical treatment, so that you will get the greatest benefit from our care. The following FAQs should answer many of your questions.
Do you take insurance?
Optimal Health Spectrums is not an in-network provider for insurance plans or Medicare, but we will provide you with a Superbill that you can submit to your insurance company for reimbursement. Some insurance carriers may cover medical services and laboratory tests performed by the provider. You will pay for your visit at the time of service and can request reimbursement from your insurance company afterward.
What type of payment do you accept?
We accept the following:

- Master Card
- Helpcard (please refer to the website main page on how to apply for this card)
- Care Credit
Many patients keep a credit card on file with our office. Payment is rendered at the time of service.
How can I prepare for my first visit?
After you have read the relevant sections of our website that pertain to your health issues, and have read “What is Integrative Medicine”, our “Practice Policies”, and this document, please click on the “Become a New Patient” button to send us your information and request an appointment. After we receive your appointment request, we will contact you within 1-2 business days to set up your new patient appointment, answer any additional questions, and collect a 50% refundable deposit to hold the appointment. We will also send you an email invitation to sign up for ChARM – our electronic medical records system.
Please fill out all the relevant online forms in your ChARM portal no later than 7 business days prior to your appointment. This is extremely important and may result in canceling your appointment, and forfeiting your deposit if this step is not followed. It is imperative to the success of your appointment that your provider is able to review your ChARM intake forms several days prior to your first visit.
Also, upload any recent (last 6 months) labs and relevant medical history notes into your ChARM portal. Always include a complete list of every supplement, prescription or over-the-counter medication, hormone, herb, etc. that you are taking, and include the ingredients and milligrams of the dose. If it is a multi-ingredient bottle or you are not sure how to list it, you may take a photo of the label and upload that.
Do I have to see the provider in person for my medical consultation?
Yes. The first and second appointment are in-person, and there must be at least one in-person appointment per year to remain an active patient. Follow-up appointments can be arranged by phone or in the office.
Can all the tests I need be done at Optimal Health Spectrums?
Most of the testing can be performed at our Optimal Health Spectrums Laboratory in Suite C. A new patient blood draw will be scheduled at the time of your new patient consultation. Some testing can be done through conventional laboratories and others are only available through specialty laboratories. During your medical consultation, your provider will determine which tests are needed and then our laboratory staff will review testing recommendations, instructions and costs. Your financial resources, and how much testing you want to do, are taken into account and the plan for testing is reviewed with you prior to implementing it. While the testing gives a much more complete picture of your health status, partial care can often be implemented without every test up front, and testing can be done over time as well. You should not let this prevent you from seeing one of our providers.
Your provider may recommend take-home test kits to collect urine, saliva, or stool, which will be discussed in detail at the time of your check-out with one of our Patient Care Coordinators. Testing is frequently done to assess nutritional status including amino acids, fatty acids, oxidative stress, vitamin levels, underlying viruses, mitochondrial function, food allergies and heavy metals. Other tests available are: genetic testing, hormone levels, bone health, gastrointestinal health, adrenal function and many others! Occasionally, we may recommend certain tests that are not performed at our facility (i.e. bone density, sleep studies, stress test, ultrasound, etc.). In those instances, we can provide you with an order that you can take to a facility near your home.
Do the providers prescribe medication?
Yes, our providers can, and do, when the medication is necessary. They are fully licensed and board-certified.
How does your integrative psychiatry treatment work?
Optimal Health Spectrums offers a wellness approach to integrative psychiatry, which means we will look into the underlying causes of your mental imbalances. Brain health begins with total body health. Dr. Mielke is a Board-Certified Psychiatrist and Nurse Practitioner Sandi has extensive psychiatric training as well, but we recommend that you have a regular primary care psychiatrist that you see for ongoing treatment for counseling, and in case of a psychiatric emergency. All integrative psychiatry patients are required to have a 90-minute initial consultation, due to the complexity of the issues involved.
Can any of your providers be my Primary Care Physician (PCP)?
No. Patients who see one of our providers should also have a Primary Care Physician (PCP). We are integrative wellness specialists and will work with you closely in preventive, nutritional and functional medicine to help you address the roots of your chronic health problems. Our providers can confer with your primary care doctor if needed.
Can you help me with my health problem?
Our providers use an innovative systems approach to assessing and treating your health care concerns. Perhaps you have experienced being examined by your doctor, only for your doctor to report back and say “all your tests are normal.” Yet, you know you are sick! Unfortunately, this is far too common.
Dr. Mielke and Sandra Kreizenbeck, FNP have helped many patients recover from current health challenges when other practitioners failed to do so, as well as helping to prevent future health issues. In addition to the basics such as optimal sleep, diet, and exercise, the “secrets” to long-lasting health and wellness involve optimizing all hormone and nutrient levels. Our practitioners also make sure that we are treating underlying stealth infections and toxins that can rob you of your vitality. In other words, when the body has all the hormones and nutrients it needs and it isn’t being infected or poisoned, it is usually happy and healthy - which is the natural state that all people should be in!
Can the provider answer questions via email?
Established patients are welcome to securely message their provider with brief, simple questions in ChARM between appointments. If a patient is having many issues or questions, it may require an appointment to review them and modify the treatment plan accordingly.
What happens once my first appointment is scheduled?
The initial office visit is a comprehensive one-on-one consultation during which your provider will work to obtain a complete picture of your, or your child’s, present condition as well as review your medical history, past and current health concerns, nutrition and lifestyle. After assessing your health needs, your provider may recommend nutritional supplements, bioidentical hormones, dietary modifications, additional testing, or other interventions. The initial office visit takes 30-90 minutes, depending on the complexity of the patient’s health situation.
The first follow-up visit is generally scheduled for 2-4 weeks after the initial visit to review any lab work received and to review your initial treatment plan. Subsequent follow-ups will be determined by your provider depending on the health situation of the patient.
What do I need to bring to the initial appointment?
If you have entered everything into your ChARM portal, you won’t need to bring anything other than some paper to take notes, or a recorder to record the appointment if you feel like you will need to review it again. You should list all of your current medications and/or hormones and supplements, with the doses in milligrams, in your intake forms. You can also make a copy of the labels and upload those into ChARM. You can also bring them in their original containers to your appointment. Any recent blood work and medical records from the last 6 months should be uploaded to your ChARM portal prior to your appointment, so that the practitioners can review them, or you can bring them to your first appointment.
What is the new patient cancellation policy?
In order to ensure that we have room in our schedule for all our patients, we have a 48 business hour cancellation policy. New patients will be asked for a credit card deposit of 50% of the appointment fee when you schedule to hold your spot. That deposit will then be applied to your appointment fee at your check-out. If you cancel within 48 business hours of your new patient appointment, you forfeit your deposit.
If you call to cancel prior to two business days before the appointment, there is no fee for canceling. You will not be charged unless you do not show up for your appointment and have not canceled within the cancellation period.

Where are you located?
Optimal Health Spectrums is conveniently located in Pleasanton, California which is at the intersection of two major freeways in the East Bay, the 580 and the 680. Our office is in the Stoneridge Office Center complex at the corner of Santa Rita and Stoneridge Drive. We are not located near the mall, but are on the opposite end of Stoneridge Drive. The main Biomedical treatments and appointments with the practitioners are all located in Suite A. Phlebotomy and IV services are in Suite C. The Optimal Health Spectrums Medispa is in Suite B.
Your location is far away from me, what do I do?
You only need to come to the office in person for the first and second appointment, and then once per year. After that, you can do your follow up appointments by phone. Most patients are able to make this work for them.
